In Connecticut, if you have an Individualized Education Program (IEP), you can begin looking at a transition plan after your 15th birthday. You can use an IEP checklist to think about schoolwork and training after high school for employment, independent living, and community participation. For Parents
If your student has an Individualized Education Plan (IEP), it's
important that you guide the IEP Team and work together to develop a
coordinated set of activities that will chart a course for the
future. In Connecticut, transition plans for Special Education
students start at least by the first IEP Team meeting after the
student's 15th birthday. Activities may explore career awareness and
employment, self-advocacy, organization and study skills,
independent living, and community participation. For more
