Working with an IEP Team
In Connecticut, if you have an Individualized Education Program (IEP), you can begin looking at a transition plan after your 15th birthday. You can use an IEP checklist to think about schoolwork and training after high school for employment, independent living, and community participation. Above all, work with your team and your family to plan. It's your future! Find more information in the Building A Bridge transition manual.
Self-Assessment
Even if you don't have an IEP team, you can still plan your future. It's important for you and your family to work together with your school, friends, relatives, other contacts, and professionals to explore the things that interest you and use your strengths. Most high school guidance offices have computer programs and other resources to help you think about what you want to do for work.
